Blackhawk Health Center

Black Hawk Health Center, operated by the Sac and Fox Nation in Stroud, Oklahoma, provides comprehensive health and wellness services for eligible Native American and Indigenous patients. Services include primary medical care, behavioral health and substance use support, dental care, optometry, pharmacy services, physical therapy, radiology, nutrition and dietitian services, podiatry, diabetes prevention and treatment, community health services, and transportation assistance. The health center also provides preventive care, health screenings, chronic disease management, wellness education, and referrals for specialized medical care.

Patient registration is available to enrolled members of federally recognized tribes who live within the health center’s jurisdictional service area. Proof of tribal affiliation and residence may be required.
